In order for you to take part in our MidnigHT walk you need to be registered. YoU CAN obtain a registration form BY clickING on the link below or please email FUNDRAISING@STLUKES-HOSPICE.CO.UK and request that one be sent out to you. Include your name and address and how many forms you require, remembering that each walker needs to complete an individual form. You may also call fundraising on 01606 555688, 01606 555684 or 01606 555687 Monday to Friday 9am to 5pm.

Boring but Important Bits!

• Places are limited and entries are strictly processed in the order we receive them.

• One form must be filled in for each person

• One cheque must be received with each entry. Entrant's name must be written on the back of the cheque.

• If you are unable to walk you must inform us as soon as possible and you must return your sponsor form even if it has no sponsorship entries. You must not 'give' your place to another person. (However your place can be allocated to your contact via the fundraising office).

• The registration fee covers all expenses involved in administering your place and is not refundable.

You are not permitted to use any of the sponsorship money you raise in relation to your entry in the Midnight Walk to pay for expenses. You must pass all of the sponsorship given to you directly to St Luke's (Cheshire) Hospice.
